What 'leaky gut' actually means
The gut barrier is a complex structure consisting of mucus layers, the intestinal epithelial cell monolayer connected by tight junctions, and the underlying immune and structural components. The tight junctions are regulated structures that permit controlled passage of specific molecules while blocking others. 'Leaky gut' is the popular term for what's more accurately called increased intestinal permeability — a state in which the tight junctions are more permissive than normal.
This is real biology. Intestinal permeability is regulated by signaling molecules including zonulin, and pathological increases in permeability are documented in multiple recognized conditions — celiac disease, inflammatory bowel disease, NSAID injury, and critical illness. The clinical and research literature establishes that the biology is genuine.
Where the controversy lives
The dispute between mainstream gastroenterology and the functional medicine ecosystem isn't about whether intestinal permeability is real biology — both sides acknowledge it where it's clearly established. The dispute is about its clinical significance outside those contexts, and about the interventions marketed as treating it. The functional medicine position has often positioned leaky gut as a primary driver of a wide range of conditions; the mainstream position has been that outside recognized conditions, increased permeability hasn't been definitively established as a clinically significant driver of systemic disease. Both positions have merit.
Where increased permeability clinically matters
In recognized conditions where increased permeability is well-established, addressing the underlying condition addresses the permeability. In post-infectious states, increased permeability often persists for months and contributes to ongoing symptoms.
